Your expertise will guide families through challenges, providing essential support and interventions that range from resource identification to short-term counseling and crisis intervention.

Your Responsibilities

In this rewarding role, you will :

• Conduct comprehensive psychosocial assessments to identify emotional, social, and environmental strengths and challenges.
• Collaborate with interdisciplinary teams to deliver holistic care.
• Develop bereavement plans to support family members and caregivers.
• Educate patients and families, ensuring understanding and overcoming barriers to care.
• Document assessments and interventions accurately in electronic medical records.
• Facilitate patient decisions regarding self-determined life care choices.
• Respond compassionately to the needs of patients and families at the end of life.

What We’re Looking For

To thrive in this role, you should have :

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois

Bachelor’s Degree in Social Work

Working Conditions

Compensation and Benefits

The compensation range for this position is $29.29 - $50.38 per hour , based on experience and qualifications. We offer a comprehensive benefits package designed to support your well-being and career growth.
