Intake & Assessment Complete intake forms and diagnostic assessments. Ensure timely and accurate clinical documentation. Complete CANS assessments using TCOM principles. Develop, update, and monitor individual treatment plans (ITPs) based on assessments and CAT / CANS. Provide crisis intervention and safety planning as needed. Participate in mandatory clinical supervision and team case consultation meetings. Participate in family collaborative intake meetings. Provide client-centered, strength-based therapy in collaboration with families. Deliver individual, group, and family therapy aligned with treatment goals. Meet direct service and documentation requirements. Support discharge planning and post-care coordination. Coordinate referrals and collaborate with families, schools, and providers. Attend supervision, team meetings, and required trainings. Maintain compliance with all clinical and regulatory standards Communicate client needs with care teams, families, and community partners. Ensure confidentiality and HIPAA compliance. Master’s degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields required Fully licensed in Minnesota as an LICSW, LMFT or LPCC Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements Related residential or human services experience working with youth and families Innovation : Leading the way and implementing creative, cutting-edge ideas and approaches. Nexus-FACTS is excited to add a full time or part time hybrid Community Based Therapist for its new 180 Degrees program! This is a hybrid, community-based position between two shelter locations (Brittany’s Place and Hope House) and the Mendota Heights office with some work remotely at home.

Location, Schedule and Pay :
- Full-time, 40 hours per week OR part-time evenings / weekends
- Schedule Monday-Friday, 10am-6pm or 11am-7pm generally for FT or evenings / weekends for PT
- Fully licensed candidates (LPCC, LMFT, LICSW) : $70-80k yearly for FT, $33-38 per hour for PT
- Mendota Heights, MN
- This is a hybrid position requiring in person, remote, and travel between sites

Using a trauma-informed, strength-based approach, the therapist delivers individualized care through evidence-based models like EMDR, DBT, TFCBT, and Systemic Family Therapy.
