Job Description :
(S.E.E.) Focused Caregiver supports the implementation of individualized life enrichment plans for residents with dementia. This role focuses on enhancing residents’ emotional and social wellbeing, assisting with personalized activities, and providing supportive care through the transitions of dementia. (S.E.E.) Focused Caregiver works closely with Transitions Coordinator to ensure a high quality of life for residents while helping maintain a positive, nurturing environment.
Key Responsibilities :
- Assist in implementing individualized life enrichment plans tailored to each resident’s cognitive abilities, preferences, and needs.
- Facilitate and group and one-on-one activities designed to promote social interaction, emotional engagement, and a sense of purpose.
- Observe and chart residents’ participation in activities and provide feedback to the Coordinator for adjustments as needed.
- Support residents during transitions between daily activities.
- Helping them adjust to changes in their environment, abilities, or routines as they transition through the stages of Dementia
- Accurate records of residents’ participation in activities and their emotional responses.
- Provide daily or weekly reports to the Coordinator regarding residents’ progress or concerns.
- Assist with traditional caregiving responsibilities if needed.
