New York RN Senior Care Manager

A company is looking for a Senior Care Manager (RN) - Transition of Care.

Key Responsibilities

Develops and assesses ongoing care plans and collaborates with providers to address members' needs

Coordinates care between members, families, and care providers to ensure person-centered services

Monitors member status and revises care plans as necessary to meet goals and needs

Required Qualifications, Training, and Education

Active New York State Registered Nurse License

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

4 - 6 years of related experience

Experience in care management and collaboration with healthcare providers

Knowledge of state and federal regulations related to healthcare services
