Registered Nurse Care Manager

A company is looking for a Registered Nurse Ambulatory Care Manager (RN) - Population Health.

Key Responsibilities

Provide clinical care management services to eligible patients, coordinating care for improved health outcomes

Develop and implement care plans, perform medication reviews, and assist with advanced care planning

Collaborate with healthcare providers and manage patient outreach and referrals to resources

Required Qualifications

Associate's Degree in Nursing (required)

Bachelor's Degree in Nursing (preferred)

RN compact license or Multi-State License (required)

2-3 years of acute care, home health, or case management experience

Case Management certification (preferred)
