Join our team as a Nurse Practitioner in a skilled nursing facility located in Brooklyn, NY.
This full-time position requires providing comprehensive medical care, conducting patient assessments, and ensuring accurate documentation.
The role involves working 40 hours per week, five days a week, utilizing electronic documentation and general office software.

Requirements

Provide medical care in a skilled nursing facility setting.

Conduct regular patient assessments and manage chronic and acute conditions.

Ensure accurate documentation of patient information.

Collaborate with interdisciplinary staff to develop care plans.

Monitor and respond to acute changes in patient conditions.

Key Responsibilities

Perform in-person clinical visits regularly and as needed.

Review and document patient medical histories.

Conduct clinical assessments of long-term care residents.

Ensure accurate documentation of clinical encounters for billing and compliance.

Qualifications

Current and valid license and registration as a Registered Professional Nurse and Nurse Practitioner.

3 years of clinical nursing practice in home care, skilled nursing facility, or hospital.

3500 hours as a Nurse Practitioner with NPI # and ability to bill Medicaid and Medicare.

Experience with electronic documentation and general office software.
