Summary : Responsible for overseeing and coordinating the activities of the case management team. This position plays a crucial role in ensuring effective patient care coordination, resource utilization, and discharge planning to optimize patient outcomes and enhance healthcare delivery. Experience :
Experience in case management within a healthcare setting, with some experience in a supervisory or leadership role. 3-5 years required
Knowledge, Skills and Abilities :
Strong knowledge of case management principles, patient care coordination, and utilization management.
Familiarity with healthcare regulations, privacy laws, and compliance standards related to case management.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work collaboratively with multidisciplinary teams and external stakeholders.
Proficiency in using healthcare information systems and electronic health records (EHRs).
Understanding of healthcare reimbursement processes and insurance coverage.
