: \n\nThe Case Manager is a Registered Nurse (RN) who has graduated from an accredited school of nursing and is currently licensed to practice in the state of Nevada. She / he supervises home health services to patients / clients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care. Skilled nursing and other therapeutic services are also under the supervision and direction of the Case Manager. The Case Manager is available at all times during operating hours and participates in all activities relevant to the professional services furnished, including the development of qualifications and assignment of personnel.\n\n : \n\n1. Registered by the state of Nevada.\n\n2. Two (2) years of experience in health care / home care required, three (3) to five (5) years preferred;\n\n3. Two (2) years' experience in a supervisory position, preferred.\n\n4. Knowledge and ability to apply community health principles and practices.\n\n5. Knowledge of agency policies and procedures.\n\n6. Ability to supervise, guide and develop skills and performance of service personnel.\n\n7. Ability to exercise independent judgment.\n\n8. Ability to work with individuals.\n\n9. Ability to enlist cooperation of many people in furthering a program.\n\n10. Ability to assist in evaluating personnel per agency policy.\n\n11. Monitors probationary period for new employees.\n\n12. Ability to deal effectively with high levels of stress.\n\n : \n\n13. Participates in developing standards which ensure safe and therapeutically effective service to patients / clients and responsible parties / legal representatives. Has joint responsibility with the Administrator for seeing that standards are met.\n\n14. Participates in developing objectives for agency.\n\n15. Is responsible for seeing that objectives are implemented.\n\n16. Consults with the Administrator to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.\n\n17. Participates in planning for the orientation of new employees. Conducts selected orientation classes.\n\n18. Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, accrediting bodies, state, etc. laws and implements such; revises concurrently.\n\n19. Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies; establishes criteria and procedures for selection, promotion and termination of employment of service personnel.\n\n20. Participates in establishing the functions and qualifications for each service position, and coordinates patient / client care and referrals.\n\na. Assures ongoing assessment of patient / client / responsible party / legal representative needs and implementation of interdisciplinary team plan of care.\n\nb. Assures physician approval of plans for continuity of care.\n\nc. Provides individual or group support concerning job related stress or issues.\n\n21. Organizes the agency to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.\n\n22. Participates in coordinating agency's services with services of other community agencies.\n\n23. Participates in studies and research and other administrative functions as assigned.\n\n24. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.\n\n25. Observes confidentiality and safeguards all patient / client related information.\n\n26. Accepts responsibility for regular attendance and punctuality and fulfills job requirements without regard to time involved.\n\n27. Serves as a resource person to employees.\n\n28. Develops a cooperative relationship and communicates effectively and professionally with the physicians.\n\n29. Investigates and reports any problem relating to patient / client care or conditions which might harm the patient / client and / or employee well-being.\n\n30. Immediately reports any accident, incident, lost articles or unusual occurrence to the Administrator.\n\n31. Attends pertinent continuing education programs other than routine in-services and shares information with staff.\n\n32. Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.\n\n33. Oversees agency's ongoing Quality Assessment and Performance Improvement (QAPI) program.\n\n34. Responsible for overseeing development of indicators with appropriate data collection, aggregation and analysis, taking action and reporting results according to agency's QAPI plan.\n\n35. Provides twenty-four (24) hours / day, seven (7) days / week on-call coverage.\n\n36. Plans and supervises the agency.\n\n37. Assures that patients' plans of care are developed, implemented and updated.\n\n38. Conducts patient / client case conferences, in-services, staff meetings and maintains documentation; participates in community education projects.\n\n39. Ensures that all necessary supplies and equipment are available.\n\n40. Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he / she will not be available.\n\n41. Ensures compliance with federal, state, accrediting bodies, local and agency policies in all patient / client care aspects of the agency.\n\n42. Makes patient / client and personnel assignments.\n\n43. Addresses and corrects all patient / client concerns.\n\nJob Type : Full-time\n\nWork Location : In person
