IDR Healthcare is seeking a local contract Home Health Physical Therapist for a local contract job in Tellico Plains, Tennessee.

Position : Physical Therapist (PT) – Home Health

Location : Tellico Plains, TN

Start Date : ASAP

Contract Type : Contract-to-Hire

Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM

Weekend Requirement : None

On-Call : Not required

Join a compassionate home health team delivering skilled physical therapy to patients in their homes. This is a great opportunity for an independent, patient-focused Physical Therapist to work with a supportive interdisciplinary care team while making a direct impact on patient recovery and mobility.

🧠 Responsibilities & Role Overview

  • Evaluate and treat patients based on physician referrals using appropriate assessment tools
  • Create and implement individualized treatment plans with measurable goals
  • Reassess and update care plans as patient needs evolve
  • Communicate effectively with physicians, caregivers, and the broader care team
  • Assist with procurement and instruction on adaptive equipment as needed
  • Supervise Physical Therapist Assistants and Aides according to state guidelines
  • Submit accurate documentation of evaluations, treatments, and billing per required timelines
  • Adhere to professional practice standards, HIPAA, and company compliance policies
  • Participate in internal staff development and maintain continuing education as required

    • ✅ Requirements

  • Active Physical Therapy license in the state of Tennessee
  • Previous home health experience strongly preferred
  • OASIS experience preferred
  • HomeCare HomeBase (HCHB) EMR experience preferred
  • AHA BLS certification
  • Excellent communication, documentation, and time management skills
  • Must be comfortable working independently in a home-based setting

    • No weekend or on-call required

    Field-based position – not remote

    Professional attire required; scrub color not specified

    This is a contract-to-hire opportunity for the right candidate
