Referral Coordinator

As a referral coordinator for Community Health Partners, you will be essential to patient experience. Community Health Partners part of Community Health System helps primary care physicians and specialists manage their practices. In your role, you will be responsible for coordinating the referral and authorization process for physician practices and serve as a liaison between the patient / family, physicians, treatment team and insurance plans to ensure quality services are being provided and patient needs are being met.

Responsibilities

In your role, you will :

• Assist the practice physician(s) and patients by coordinating and tracking insurance authorizations and referrals for outside testing or specialty care.
• Submit referral activity reports to supervisor for review and follow-up if necessary.
• Coordinate and communicate with various insurance carriers to obtain necessary pre-authorization for patients to receive necessary services.
