A company is looking for a Member Health Assessor to engage with members and conduct health risk assessments.
Key Responsibilities
Conduct health risk assessments by gathering information from members and relevant stakeholders
Coordinate care plan logistics and contribute to the development and implementation of care plans
Maintain documentation and improve processes to enhance member experiences
Required Qualifications, Training, and Education
High School Diploma or General Education Diploma (GED) is required
Minimum of two years of experience in community settings with at-risk populations is preferred
Proficiency in Microsoft Office, including Outlook, Word, and Excel
Sensitivity to and experience working within different cultures
Ability to work independently and collaboratively in a team environment
