This role requires skilled nursing care in a home setting, focusing on patient assessments, OASIS documentation, and coordination with healthcare professionals to ensure quality patient outcomes.

Key Responsibilities :

• Perform in-home assessments and develop personalized care plans
• Provide wound care, IV therapy, and medication administration
• Ensure timely and accurate OASIS documentation (Homecare Homebase experience preferred)
• Educate patients and caregivers on treatment plans and health management
• Work autonomously while collaborating with a multidisciplinary team
