- MDS Assessor (Registered Nurse)
- for a full-time contract role. This position offers consistent weekday hours, competitive pay, and a supportive team environmentno weekends or holidays required.
As an MDS Assessor, youll be responsible for coordinating and completing resident assessments and Minimum Data Set (MDS) documentation in compliance with State and Federal regulations. Youll play an essential role in ensuring accurate reporting, optimizing reimbursement, and supporting quality care outcomes for residents.
- Duties Include :
- Conduct and coordinate comprehensive resident assessments in accordance with facility and regulatory standards
- Review the daily census to identify residents requiring MDS assessments
- Prepare interim and significant change assessments for Medicaid and Medicare
- Input and transmit MDS / PRI data accurately and on time
- Generate Quality Indicator reports and share findings with leadership
- Maintain organized MDS booklets, documentation, and resident records
- Collaborate with interdisciplinary teams and Quality Improvement staff
- Order and manage MDS office supplies as needed
- Perform additional duties related to compliance and data accuracy
- Qualifications :
- Active New York State Registered Nurse (RN) license required
- Bachelor of Science in Nursing (BSN) required
- Minimum 5 years of clinical nursing experience
- At least 2 years of experience performing MDS assessments required
- Strong working knowledge of Medicare / Medicaid regulations and DOH transmission systems
- Excellent communication, documentation, and organizational skills
