DESCRIPTION OF WORK
Reporting directly to the Aging Care Management Supervisor 1, the Aging Care Manager 2 assesses the needs of consumers utilizing state mandated assessment forms, develops effective care plans to meet these needs, and implements care plans in a professional manner. It is essential you maintain knowledge of all AAA service programs and community support services for the benefit of older adults and maintain current listings of these services. You will assist consumers with pre-placement and placement activities when community services are no longer sufficient to maintain consumers in the community. Your organizational skills are key as you prioritize many phone calls and consumer requests at the same time, resolving issues to a satisfactory conclusion. You will have the opportunity to assist consumers in navigating the Aging network of services and offer consumers' choices of available services.
