Department : Cardiac Post Transplant Services
Schedule : Full-time, Day Shift, 40 hours per week
Hospital : Ascension Saint Thomas West
Location : Nashville, TN
Coordinates clinical and administrative processes for patients through all phases of the transplant / mechanical circulatory support (MCS) implant continuum to include pre-transplant / implant evaluation, wait list management, transplantation / implantation and post-transplantation / implantation follow-up care.
Educates patients, families and staff regarding transplantation / MCS implantation.
Collaborates with physicians and other program staff in evaluating and managing patients.
Completes data capture, analysis and reporting and other administrative functions required for transplant / MCS program operations.
Actively participates in the quality agenda for transplant services as outlined by the United Network of Organ Sharing (UNOS) and Centers for Medicare and Medicaid Services (CMS).
Collects and maintains time study data for purposes of reporting pre and post transplant time.

Responsibilities
Coordinate nursing program activities and resources within clinical specialty or department.
Develop and oversee program, making recommendations and acting as a resource for all nursing staff related to patient care and clinical standards, policies and procedures. Develop and document care plans and therapy programs.
Oversee and ensure plan of care therapy program is implemented as indicated. Ensure necessary resources and / or equipment is obtained as ordered.
May supervise and direct nursing staff.
Coordinate, assemble, and maintain documentation as required.
Requirements
Licensure / Certification / Registration :
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
One or more of the following required :
Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.
