Nurse Practitioner

Pride Health is currently hiring a part-time Nurse Practitioner for our Home Health client in Tuscaloosa, AL! In this role, you will provide direct, in-home medical care to patients within a designated geographic area, with a focus on enhancing health outcomes and improving quality of life. You will collaborate closely with primary care providers, caregivers, and other healthcare professionals to deliver comprehensive, patient-centered care.

Shift : This position requires approximately 10 hours per week, Monday through Friday. Scheduling is flexible, and hours may be distributed across the week based on candidate availability.

Pay : $64-$69 per hour

Contract Length : 3+ months / opportunity for extensions / permanent hire

Responsibilities :

• Perform comprehensive assessments and develop ongoing care plans.
• Provide follow-up care and interventions to reduce avoidable hospital admissions.
• Prescribe medications, order diagnostics, and manage treatment plans.
• Perform basic procedures such as IV placement, injections, wound care, and blood draws.
• Educate patients and families on disease management and wellness.
• Collaborate with interdisciplinary teams and participate in quality initiatives.
