Social Worker

Job Title : PRN MSW Home Health Social Worker

Location : Collier County

Job Type : PRN

Job Description :

We are seeking a PRN MSW Home Health Social Worker to join our fast-growing team. The candidate will be responsible for providing therapeutic services for patients in the home health and private duty settings. The ideal candidate will possess excellent communication skills, experience in developing care plans, and the ability to coordinate patient care with other healthcare professionals.

Responsibilities :

  • Assess patient's emotional, environmental, social and financial needs, and develop appropriate care plans.
  • Coordinate patient care with other healthcare professionals, such as physicians, nurses and therapists.
  • Monitor and evaluate patients' progress, and adjust care plans as necessary.
  • Educate patients and caregivers about resources and services available in the community.
  • Provide counseling and emotional support to patients and families.
  • Maintain thorough documentation of patient care in electronic medical records, as well as paper files.

    • Qualifications :

  • Master's degree in Social Work from a school accredited by the Council on Social Work Education.
  • Active and valid state license as a Social Worker.
  • Minimum of one year of experience in the field of healthcare, home health or hospice preferred.
  • Knowledge of community resources, and experience in navigating and coordinating care with local agencies.
  • Excellent communication skills, both verbal and written.
  • Ability to work independently, and to manage and prioritize multiple tasks in a fast-paced environment.
  • Must have a reliable vehicle, valid driver's license, and auto insurance.
