Job Summary :

Visits patients, assesses their needs, interacts with families and other hospital staff for the good of the patient. Facilitates chapel services. Responds to all codes and crisis calls.

Job Responsibilities and Requirements :

PRIMARY RESPONSIBILITIES

• Visits patients, families and staff. Prepares daily census sheets for the department.
• Prepares for chapel services.
• Coordinates all sacramental / liturgical ministry within the hospital and documents what is necessary.
• Refers patients, families, and staff to chaplains, priests and ministers, as needed.
• Sends sympathy cards to families of deceased patients.
• Participates in and plans for memorial services.
• Responds to all codes.
• Covers on call shifts as assigned.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
