OVERVIEW OF THE POSITION :
Under the direction of the Oakland DRC Casework Supervisor, the DRC Case Manager is responsible for daily provision of direct services and case management activities to DRC parolees. The DRC Case Manager is responsible for participant casework including Individual Treatment Plans (ITP), group and individual counseling, information and referral activities, and documentation of services activities. Utilizing Motivational Interviewing and Cognitive Behavioral Therapy techniques, the DRC Case Manager ensures that caseload participants are provided with the tools and skills required to remain drug and arrest free through the adaptation of pro-social behaviors. The DRC Case Manager is responsible for monitoring the progress of parolees throughout all phases of their participation.
RESPONSIBILITIES / STATEMENT OF DUTIES :
- Conducts screening and risk / needs assessment and face-to-face group and individual counseling services to DRC participants; Responsible for daily supervision of DRC participants.
- Under supervision of the Casework Supervisor, develops and implements evidence-based rehabilitative interventions based on Motivational Interviewing and cognitive behavioral therapy techniques.
- Works with participants to develop and implement ITPs for participants throughout all program phases.
- Develops individual case files to include assessment information, service plans, progress notes, participation tracking forms, discharge plans, releases of information, disclosure forms, and all required CDCR / DAPO program-specific documents.
- Provides service activities (18 : 1 ratio) as scheduled, based on indicated curriculum and daily activity schedule.
- Attends Multi-Disciplinary Team case management conferences and meetings as directed.
- Assists with monitoring of vocational services activities.
- Under the direction of the Casework Supervisor, facilitates groups and individual counseling relative to required DRC program service requirements.
- Consults and collaborates with other DRC program staff regarding participants on caseload.
- Maintains regular contact with Agents of Record regarding the progress of participants assigned to caseload inclusive of discharge and aftercare planning.
- Monitors and records participants’ Community Services requirements and activities.
- Where applicable, monitors participant progress on follow through for referrals (Domestic Violence, housing, public benefits, health care, etc.).
- Provides case management services for aftercare services; Identifies other required community-based support services and resources; Works with participant to create support network in the community.
- Documents participant progress and treatment activities through weekly progress notes and maintains accurate case files.
- Documents participant issues of non-compliance using CDCR Title 15 and other DAPO identified guidelines and procedures.
- Conducts motivational interviewing sessions with participants to convey the importance of addressing community reintegration issues specific to ITP goals and objectives.
- Provides classroom and group management and supervision of participants.
- Negotiates modification of ITPs with participant and Multi-Disciplinary Team.
- Provides assistance to the Program Director and Casework Supervisor in training of new program staff.
- Monitors participants’ progress and provides review to the Casework Supervisor; Evaluates the progress of participants on caseload under the supervision of the Casework Supervisor; Informs AOR of determinations.
- Participates in in-service training, CDCR / DAPO cross-training, and Agency Work Force Development training.
- Maintains secure storage of all DRC participant records as instructed by the Casework Supervisor.
- Seeks formal weekly supervision with Casework Supervisor.
- Assists Job Developer in tracking participant job search activities.
- Where applicable, tracks participant progress and self-study activities in obtaining GED.
- Submits participant requests to Casework Supervisor (transit vouchers, request for Caltrans Work Crew, etc.).
- Facilitates aftercare groups as directed.
- Ensures the health, welfare, and safety of program participants.
- Follows and maintains all CDCR / DAPO safety and security guidelines, policies, and procedures.
- Participates in peer chart reviews.
- Provides services to diverse participant population in a culturally sensitive manner.
- Assists with data collection activities
- Participates in regularly scheduled Center Point trainings and continuing education training conducted by approved entities.
- Observes required agency dress code and all other personnel policies.
- Other duties as assigned.
OAKLAND DRC CASE MANAGER MINIMUM REQUIREMENTS :
High School Diploma or equivalent and a minimum of six months working with and providing rehabilitative services to criminal justice populations.Basic knowledge of Cognitive Behavioral Therapy, Motivational Interviewing Techniques, Psycho-Educational interventions, and 12-Step programs.Must meet CDCR clearance requirements.If an ex-offender must be successfully discharged from parole or probation for a minimum of three (3) years prior to hire.Must have current TB clearance.Knowledge of applicable ethical and professional standards of conduct.Good written and verbal communication skillsIntermediate computer skills.Possesses a valid California Driver’s License.Demonstrated experience in group and individual counseling services, case management, documentation, treatment plan development, and the independent development of evidence-based rehabilitative services interventions to address service needs.Demonstrated experience in developing targeted treatment outcomes and strategies.Experience working with a culturally diverse population and in a culturally diverse, multi-disciplinary staff.Ability to promote culturally appropriate and gender responsive services.PHYSICAL DEMANDS- CLINICAL STAFF ENDURANCE- MOVING ABOUT :
