Per Diem Home Health Aide (Transportation)

Employment Type Part time Shift Description : Home Health Aide Location -Trinity Health PACE - New Jersey Hours - On- Call Position Purpose Trinity Health PACE is seeking a compassionate Home Health Aide to support the personal care and health needs of our elderly participants. You'll play a vital role in helping participants maintain independence and dignity in their daily lives. What You Will Do

• Provide personal care assistance including bathing, grooming, toileting, and mobility support
• Monitor and report changes in participants' physical or emotional condition
• Support participants during recreational activities, meals, and appointments
• Ensure safety and infection control standards are met
• Collaborate with the interdisciplinary care team and maintain accurate documentation
• Assist with transportation and home care services as needed Minimum Qualifications
• High school diploma or equivalent
• Active Home Health Aide certification (Certified Nurse Assistant preferred)
• CPR certification required
• At least 1 year of experience with frail or elderly populations (or willingness to train)
• Valid driver's license, car insurance, and reliable transportation required
