Job Summary

We are seeking a compassionate and organized onsite Registered Nurse - Case Manager to provide both clinical care and case management for individuals with developmental disabilities.

This role focuses on care coordination, health monitoring, and advocacy, ensuring that individuals receive appropriate and timely services.

Key Responsibilities

Coordinate medical care and community services for assigned individuals

Monitor health conditions and update care plans accordingly

Serve as a liaison between providers, families, and support staff

Educate individuals and staff on health, wellness, and medication management

Maintain accurate medical records and documentation

Supervise and support LPNs and direct care staff

Qualifications

Current NY Registered Nurse (RN) license

BSN required

At least 1 year of experience in a community or developmental disability setting

Case management experience a plus

Strong communication and organizational skills
