We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. RN : $30.00 per hour. The RN provides skilled visits in patients' homes. This position will be serving Trumbull County. Responsibilities What You'll Do : ∙ Administer medication in accordance with standards of practice. ∙ Encourage social ehabilitative skills and encourage self-care and independence. ∙ Perform client treatments in accordance with Plan of Care ∙ Assist the consumer in learning appropriate self-care techniques. ∙ Prepare clinical and progress notes to document care provided. ∙ Monitor, document accordingly and report any change in client condition to Clinical Director / Clinical Supervisor ∙ Promote activities to achieve a safe and healthy environment. ∙ Observe aseptic technique when performing client procedures / care. ∙ Collaborate with other members of the clinical team to develop and improve the Plan of Care ∙ Assist the client in achieving established goals in the Plan of Care developed by the RN. ∙ Maintain productivity according to the standards of the agency. ∙ Maintain license, CEUs and mandatory employment requirements. ∙ Participate in client care meetings, in-services, orientation and staff meetings. ∙ Provide service in accordance with agency's philosophy, standards, policies and scope of practice ∙ Participates in Performance Improvement and adheres to HIPAA guidelines. ∙ Other duties as assigned. • Valid State RN License
• Valid driver's license and use of automobile is required due to the travel involved.
• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
• Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Must be able to use a computer, laptop or tablet.
• Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
• Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served.
