Experience : Minimum years of experience preferred, new graduates are acceptable

License : Valid state license required

Certifications : Valid certifications required

Must-Have :

1. Completed prescreen form attached upon submission.

2. Resume and license verification attached.

3. SNF experience highly preferred.

4. Three references provided.

5. Completed criminal background checks (county, federal, multi-state, nationwide sex offender).

6. HHS / OIG search completed.

7. GSA search completed.

8. Social Security Number trace completed.

9. 1-step PPD or Quantiferon or proof of positive PPD and chest X-ray provided.

10. SAM search completed.

11. CORI background check completed.

12. 8 hours of dementia training completed.

Description :

New graduates are accepted, but SNF experience is highly preferred.
