Position Description
The Intake Specialist identifies referral sources in the community and actively participates in the recruitment of clients for the ADHC. The Intake Specialist also assists and evaluates new clients for eligibility.
Essential Job Functions
The following duties are mandatory requirements of the job :
1. Coordinates the intake process to ensure that descriptive information is provided to applicants and agencies; ensures that inquiries about the application and admissions process are responded to in a timely and appropriate manner. Regularly reviews referral log to determine applicant trends and percentages of applicants accepted to and attending the program.
2. Assists prospective clients in gathering medical and other documentation necessary for program enrollment.
3. Prepares monthly reports on referrals and applicants and regularly reviews referral log to determine applicant trends and percentages of applicants accepted to and attending the program.
4. Ensures that applications are logged and that follow-up for face-to-face interviews, if appropriate, are conducted in a timely manner.
5. Participates in all required ADHC team and committee meetings.
6. If necessary, assists clients in applying for entitlements or refers them to appropriate governmental agency. Follows up on status of pending entitlement applications.
7. Develops resource database of accessible services in neighborhoods and meets and provides outreach services on an ongoing basis.
8. Provides orientation group to incoming clients concerning program and supportive services available to them. Informs clients of program rules upon admission and ensures that each client signs the Admission Agreement.
9. Provides training for staff in regard to application, admission, and referral procedures.
10. Functions as primary liaison between program and outside providers (e.g., hospitals, treatment programs, social service agencies).
11. Collaborates with the Business Development Department in coordinating presentations for internal programs of Harlem United and organizations in the community.
12. Conduct outreach to members with inconsistent attendance to re-engage in services.
13. Monitor and track monthly list of new intakes for the first 30 days of program enrollment to ensure connection to program services. Outreach those clients who have attended program inconsistently.
14. Responsible for a minimum of ten presentations per month.
15. Responsible for a minimum of five admissions per month.
16. Performs other duties as required by Supervisor.
Education and Certification
An Associate’s Degree preferred, and three years’ experience working in care coordination services for consumers related to health services. Experience collaborating with individuals with health disparities such as HIV, Mental Health, Homelessness, welcomed.
