Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract role in Monroe, LA. The nurse will provide patient care in the home setting, completing OASIS documentation and managing visits through Homecare Homebase (HCHB). This role is ideal for RNs who enjoy autonomy while delivering compassionate, high-quality care.

Key Responsibilities :

Perform patient assessments, admissions, discharges, and recertifications using OASIS

Deliver direct care and education in patients homes

Document all visits accurately in Homecare Homebase (HCHB)

Collaborate with physicians and the interdisciplinary team for patient-centered care

Manage a caseload efficiently with professionalism and clinical expertise

Qualifications :

Current RN license in Louisiana

Minimum of 2 years of nursing experience, including at least 1 year in home health

Proficiency with OASIS and Homecare Homebase (HCHB) required

Valid drivers license, auto insurance, and reliable transportation

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
