We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse to join our healthcare team. The ideal candidate will provide high-quality patient care in a long-term care facility. As an LPN / RN, you will deliver essential nursing services while ensuring compliance with healthcare regulations and standards.
Setting : Short / long-term care, skilled nursing and rehabilitation facility.
Responsibilities
- Administer medications and treatments as prescribed by healthcare providers.
- Conduct patient assessments and monitor vital signs.
- Assist with daily living activities for patients, including personal hygiene and mobility.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide education to patients and families regarding health management and disease prevention.
- Participate in discharge planning and follow-up care coordination.
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
Experience
Candidates should possess the following skills and experience :
Experience in critical care, long-term care, assisted living, or home care settings is preferred.Familiarity with EMR / EHR systems such as Point-Click-CareExperience working with diverse populations including individuals with developmental disabilities, Alzheimer's care, dementia care, or geriatrics.Proficiency in patient assessment techniques, medication administration, phlebotomy, and basic life support (BLS).Previous experience in a long-term care setting is advantageous.
