Citizens Memorial Hospital (CMH) is seeking a LPC, LCSW, or Licensed Psychologist to join the team at the Bolivar Family Care Center. CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, rehab therapies and more, CMH has its community covered!

About the department : CMH's Behavioral Health Department focuses on treating the “whole” person, not just the parts. CMH's behavioral health team includes psychiatrists, psychologists, counselors and licensed clinical social workers offering services in a variety of settings. Conditions treated include dementia, depression, addiction recovery, anxiety, and loss and grief, just to name a few.

A typical day will include : Providing support and treatment to patients, family members, and caregivers in collaboration with other integrated care team members.

• Conducts assessments to determine care needs
• Develops individualized treatment plans in collaboration with medical teams
• Provides therapy to help patients cope with illness and its effects
• Connects patients with community resources, such as financial assistance, housing, or support groups
• Maintains accurate and confidential patient records within the defined reporting system

This position is : Full-time, Monday - Friday, 8am-5pm.
