Key Responsibilities (What You Will Do)
Perform comprehensive assessment, diagnosis, and evidence-based treatment of acute and chronic wounds in mobile settings.
Execute advanced wound procedures, including debridement, biopsy, and specialized dressing selection.
Collaborate effectively with physicians, nurses, and home health providers to develop and adjust seamless care plans.
Act as a clinical resource by identifying and recommending improvements in workflow, documentation, and patient care protocols using lean methodologies. We firmly believe in growing as a team and every has something to contribute.
Provide thorough patient and caregiver education on wound prevention, management, and ongoing maintenance.
Maintain meticulous, compliant Electronic Medical Record (EMR) documentation for all patient encounters.
Qualifications (What We Are Looking For)
Required :
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Active and unrestricted Nurse Practitioner (NP) license in the State of Michigan.
Minimum of 2 years of experience as a practicing Nurse Practitioner (NP).
Valid driver's license and access to reliable transportation.
Demonstrated ability to be independent, highly organized, and an excellent communicator.
Preferred :
Experience in wound care, home health, geriatrics, or palliative care.
Active Wound Care Certification (WCC, CWS, CWCN) or a strong willingness to obtain certification within the first year of employment.
