Job Description

What you will be doing : Pharmacy Clinical Specialist - Hematology / Oncology

Perform advanced clinical pharmacy duties to enable optimal pharmacotherapy delivery to patients.

Facilitate safe, efficacious, and pharmacoeconomically rational use of medications in the population(s) served.

Conduct daily reactive and targeted proactive medication regimen reviews for patients.

Support the delivery of pharmacy managed protocols, formulary processes, and medication reconciliation activities.

Provide education to fellow pharmacists, nurses, and medical staff.

Precept pharmacy students and pharmacy residents on clinical patient care rotations.

Contribute to fulfilling the organization's mission of providing innovative, personalized, and cost-effective pharmaceutical care.

Experience you will need : Pharmacy Clinical Specialist - Hematology / Oncology

Graduate of an education program approved by the credentialing body for the required credentials.

Successful completion of an ASHP-accredited post-graduate year one (PGY1) residency training program or equivalent experience, such as three years of pharmacy experience per residency year.

ASHP-accredited post-graduate year two (PGY2) or fellowship preferred.

Three years of clinical practice experience.

Availability Monday through Friday from 8 : 00 AM to 4 : 30 PM, with rotating on-call responsibility.

PHRM - Pharmacist - State Licensure Texas Pharmacist License or eligible for Texas Pharmacist License within 90 days AND

PPC- Pharmacist Preceptor Cert - State Licensure within 90 days
