A company is looking for a Social Worker Care Manager to service patients in the Queens, NY area.

Key Responsibilities

Support the mental and physical health care of patients on an assigned caseload

Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care

Document patient progress and treatment recommendations in EHR and track clinical outcomes

Required Qualifications

Minimum of MSW required

Preferred licensure or provisionally licensed

Experience and knowledge in behavioral health

Must possess a valid driver's license and have access to an insured automobile

Proficiency in using Excel and Word for daily assessments and productivity tracking
