Nationwide Therapy Group is seeking a travel Home Health Physical Therapist for a travel job in International Falls, Minnesota.

Position Information

  • Role : Therapist (PT / OT – unspecified in listing, please confirm)
  • Shift : 8-hour days, Monday–Friday, 8 : 00 AM – 4 : 30 PM
  • Guaranteed Hours : 40 per week
  • Contract Length : 13 weeks (91 days)

    • Workload & Coverage

  • Driving Radius : 40 miles
  • Average Caseload : 5 cases / day
  • Settings Covered : Home Health, Hospice, SNF (including International Falls)
  • Supervision : Yes, PTAs may need to be supervised

    • Unit / Facility Notes

  • Local rates apply within 80 miles
  • Can start earlier than 3 weeks if cleared
  • $500 backout / quit fee if 2-week notice not given

    • Requirements

  • Certifications : BLS required
  • Must have at least 1yr of recent experience in setting.
  • Experience :

    • Must have Home Health experience

  • May consider new grads if completed Home Health clinicals
  • Licensure : Must have active license to submit

    • Technology & Documentation

  • Charting System : Remote Client
  • Mileage Reimbursement : Yes, at government rate
  • Drive Time : Billable
