PACE KC Home Health Aide

The Home Health Aide provides quality care to participants, under the supervision of the Home Care Coordinator and nurses in accordance with PACE KC policies and procedures. Performs related tasks in accordance with the participant's plan of care. Maintains flexibility in schedule and responds to unexpected Participant care needs and changes in workload and location of work as directed. This position is supervised as designated on the agency organizational chart.

Primary Functions :

• Possesses effective communication skills to act as a companion and provide emotional support to participants and caregivers.
• Possesses interpersonal skills that promote interdisciplinary collaboration and communication.
• Identifies need for intervention by other members of the IDT on an ongoing basis and reports to primary nurse.
• Provides input into interdisciplinary evaluation of the effectiveness of care
• Performs and / or assists the patient / family with home maintenance activities as according to Plan of Care including light housekeeping and meal preparation.
• Performs support activities as indicated on the plan of care including but not limited to supporting independence, providing companionship, and providing respite for families
• Establishes therapeutic relationships with participants and families from a variety of cultural, ethnic and socioeconomic backgrounds.
• Provides emotional support to participants and families in a manner that fosters independence and affirms existing strengths.
• Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
• Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
• Participates in in-service training and staff meetings as required.
• Performs other duties as assigned.

Certification & Licensure Requirements :

Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or obtain within two weeks of hire, before having direct participant contact.
