Are you a skilled and compassionate professional ready to make a meaningful impact in the lives of others? LifeSpan Home Care , a trusted name in home health services, is seeking a PAS Case Manager to join our dedicated team in Fort Worth, TX .

About the Role :

As a Case Manager , you’ll play a vital role in coordinating and supervising personal assistance services across a variety of programs, including :

  • STAR+PLUS
  • Primary Home Care
  • Family Care
  • Community-Based Alternatives
  • MDCP
  • Private Duty / Personal Assistance / Sitter Services

You’ll ensure high-quality care is delivered in accordance with physician plans, state program guidelines, and agency policies—while also providing hands-on support when needed.

Key Responsibilities :

  • Supervise PAS / community care staff and coordinate patient care
  • Manage billing, scheduling, and office operations
  • Conduct timely supervisory visits and maintain accurate documentation
  • Collaborate with HR, payroll, and external caseworkers
  • Ensure compliance with regulatory and agency standards
  • Support hiring, training, and performance tracking of care staff
  • Participate in budget planning, meetings, and quality improvement initiatives
  • ? What We’re Looking For :
  • 2+ years of supervisory experience or 2 years of college coursework
  • Experience in PAS program management preferred
  • Strong communication, organization, and leadership skills
  • Knowledge of home care regulations and Medicaid / managed care programs
  • Proficiency in MS Word, Excel, and agency-specific software
  • Ability to type 35 WPM with 80% accuracy
  • Bilingual (English / Spanish) preferred
  • Reliable transportation, valid driver’s license, and auto insurance required
