Summary
The Palliative Care Nurse is an integral part of the care team necessary to facilitate coordination in care, education, and to provide ongoing support to the IDT as well as patient / family regardless of care setting (i.e. inpatient, home, or clinic).

Qualifications
- RN licensure in the state of SC
- Office experience preferred
- Previous experience and training in Palliative Care
- Experience in the care of patients with advanced illness
Duties / Responsibilities
Serves as a member of the Inpatient and Community Palliative Care TeamAvailable as resource for patients, families and medical community regarding palliative care and end of life careParticipates in regularly scheduled meetings of Palliative Care TeamAssists in management and scheduling of Palliative Care referrals for both inpatient and outpatient careAssists in scheduling of family care conferencesAssists with in-services and education for physicians and the community on Palliative CareCollaborates and assists hospital case managers with discharge planning on referred patientsProvides required documentation of services providedProvides staffing for the Supportive Care ClinicCoordinates and assists physician and Nurse Practitioner in Palliative Care ClinicConducts principal care management follow up calls and provides updates to nurse practitioners / IDT as necessaryPerforms prior authorizations as needed on medication, services provided within the clinicAssists with data collection and reports related to services provided by Palliative Care ProgramSupports the overall culture of the organizationParticipates in organization sponsored eventsPerforms related duties as assigned or as needs ariseSubject to a post employment background check and drug screening as a condition of employment.
