Social Services Specialist III

This position is located in Oklahoma City, Oklahoma.

Social Services Specialist III H20C Annual Salary :

• $43,528.94 + Full State Employee Benefits Travel is OCCASIONAL.
• Must possess a valid drivers license and must maintain required car insurance.
• Minimum Qualifications Education and Experience requirements at this level consist of a master’s degree plus two additional years of qualifying experience; or a bachelor’s degree and three years of experience in professional social work; or an equivalent combination of education and experience, substituting one additional year of experience in professional social work for each year of the bachelor’s degree.
• Job Responsibilities This Social Services Specialist III provides comprehensive HIV case management to individuals & families living with HIV, with a targeted outreach emphasis on at risk and under served populations throughout Oklahoma.
• Also, facilitates & provides access to federal & privately funded prescription assistance programs, specialized home health programs, COBRA benefits, & medical insurance; Initiate and facilitate expedited Medicaid Waiver services for clients with urgent medical needs.
• Provide comprehensive services plan development with the client in conjunction with APS, CWS, and AFS agency staff.
• Refer & schedule client appointments for medical services; Serve as gatekeeper for client access to emergency community funds and pantries.
• Collaborate with other agencies through monthly inter agency staff meetings to identify, develop, and maintain client services.
