Geriatrics Nurse Practitioner

We are seeking a compassionate and experienced Adult Nurse Practitioner to join our team in providing primary care services to residents in assisted living facilities throughout New York.

What You ll Do :

Provide comprehensive assessments , physical exams, and medication reviews

Create individualized care plans in coordination with an interdisciplinary team

Manage chronic and acute conditions

Order and interpret labs and diagnostic tests

Prescribe medications in accordance with NY State guidelines

Educate patients and families on health maintenance & disease prevention

Participate in chronic care management programs

Maintain accurate documentation using eClinicalWorks (eCW)

Requirements :

Master s degree in Nursing and completion of an accredited NP program

Unrestricted NY Nurse Practitioner license

Valid DEA license

Eligible for Medicare / Medicaid reimbursement

3+ years of clinical experience (3,600 hours), ideally in long-term care or geriatrics

Locations : Multiple sites across New York State on-site presence required.
