- Board Certified in OB / GYN - REQUIRED
- Board Eligible (to become Board Certified within 2 years of becoming Board Eligible) or Board Certified in Maternal Fetal Medicine - REQUIRED
- MI license (or willing to obtain) - REQUIRED
- DEA (or willing to obtain) - REQUIRED
- BLS, ACLS (or willing to obtain) - REQUIRED
- Clean malpractice - PREFERRED
- Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
- ACLS
- BLS
- Board Certified

- DEA
- Michigan
