Responsibilities
Assist in creating and implementing education curriculum with Clinical Education department.Demonstrates techniques and best practices for trainees.Assist in creating and implementing policies and procedures for all clinicians.Initiate appropriate preventive and rehabilitative nursing procedures.Administer medications and treatments as prescribed by physicians.Conduct initial assessments of patients and families to determine home care needs.Perform thorough physical assessments and gather comprehensive medical histories.Develop and revise individualized plans of care based on patient status and evolving needs.Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.Responsively address inquiries and concerns from patients, caregivers, and medical teams.Provide counseling and support to patients and caregivers before, during, and after treatment.Supervise home health aides as required.Identify discharge planning needs and proactively implement them as part of the care plan development.Qualifications
