$35.16 - $54.50 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Patient Care Navigator, Oncology Infusion Center

Position Highlights :

• Position : Patient Care Navigator
• Location : Northwest Community Hospital. Arlington Heights, IL.
• Full Time : 40 hours / week
• Hours : Monday-Friday, (8a-4 : 30p)
• Required Travel : 3040 W Salt Creek Ln. Arlington Heights, IL.

What you will do :

Reporting to the Service Line or specialty program leader (Cardiovascular, Neuroscience, Cancer, GI, Orthopedic, Geriatric), the Patient Navigator serves as the primary point of contact for the patient through the continuum of care; diagnosis, planning, treatment, pre and post-operative care, discharge and follow up. The Patient Navigator functions as a resource to patients, seeking patient input into the plan of care. The Patient Navigator engages and collaborates with all appropriate care givers (physicians and the entire care team) in the plan of care, while serving as the advocate for the patient. The Patient Navigator provides patient and family education, and access to hospital and community resources as well as community education and screening.

Facilitates patient appointments, communication with the physicians, and support services (Social Work, PT, etc.). Identifies and addresses the clinical and service delivery barriers to care.
