Phoenix Rising Mental Health Outpatient and Transitional Living is looking for Mental Health Workers!
This position is responsible for providing direct service to clients, and for facilitating the smooth day-to-day operation of the program. This position collaborates as a member of the multidisciplinary team to deliver effective treatment and support.
AVAILABILITY : We are currently looking for the following
- Tuesday-Saturday, Sunday-Thursday, Friday-Monday
- All shifts : 7am-3 : 30pm; 3pm-11pm; 11pm-7 : 30am
Essential Duties and Responsibilities
Assist with intakes and orient clients to the facilityConduct and document rounds at their scheduled frequency to ensure the location and well-being of each client.Support clients with medication management as needed.Build rapport and provide support to clients while maintaining appropriate boundaries at all times.Transport and attend offsite activities with clients such as meetings, appointments and therapeutic outings.Ensure clients attend all scheduled groups and therapy sessions.Prepare and facilitate groups as assigned and lead clients in organized activities.Review treatment plan goals with clients regularly and support clients in accessing community resources.Provide effective crisis intervention as needed and teach independent living skills effectively.Complete all charts, program documentation, shift / crossover reports, incident reports and other forms and reports according to company policies.Document and communicate to the treatment team any pertinent observations and interactions with clients.Ensure the outpatient facility and transitional living is clean, safe and in compliance.Attend in-service trainings and all-staff meetings to complete 20 hours of continuing education per year.Other duties as assigned by the leadership team.Qualifications
At least 18 years of age.High School graduate or possess a GED. Coursework in counseling, psychology, or social work preferred .Experience working directly with mental health required .Demonstrated ability to work independently and as a team player. Excellent communication skills both oral and writtenValid driver's license, clean driving record and personal automobile insurance.Benefits :

Hiring is contingent upon completion of a background check, TB test, and clearance on a drug screen and pre-employment physical.
