Location : Santa Fe, NM, 87505

Duration : 32-week contract

Shift : 08 : 00 AM - 04 : 00 PM

Guaranteed Hours : 40 per week

Weekly Pay Rate : $1,920 - $2,120; (Negotiable based on experience)

Job Description :

We are seeking an experienced Registered Nurse Case Manager for a contract position at a healthcare facility in Santa Fe, NM . The ideal candidate will have a strong background in discharge planning , care coordination, and interdisciplinary collaboration.

Key Responsibilities :

• Coordinate patient care across the continuum, focusing on safe and effective discharge planning
• Collaborate with physicians, nursing staff, social workers, and other healthcare professionals to develop individualized care plans
• Evaluate patients' medical, psychosocial, and financial needs to determine post-acute care requirements
• Facilitate communication between patients, families, and care teams to ensure understanding of discharge instructions and follow-up care
• Identify and address barriers to discharge, including insurance issues, home care needs, and transportation
• Document case management activities accurately and in compliance with regulatory standards
• Monitor patient progress and adjust care plans as necessary
• Ensure continuity of care by coordinating with external agencies, skilled nursing facilities, home health, and other community resources

Required Qualifications :

At least 3 years of clinical nursing experience.

BLS (Basic Life Support)
