A company is looking for a Social Worker Care Manager.

Key Responsibilities

Support mental and physical health care for assigned patients

Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care

Document patient progress and treatment recommendations in EHR and track clinical outcomes

Required Qualifications

Minimum of MSW

Preferred licensure or provisionally licensed

Experience and knowledge in behavioral health

Must have a valid driver's license and insured automobile

Time management skills are essential
