QUALIFICATIONS :
- Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital or med-surg clinical rotation in a nursing school or have at lease one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course approved by the Vice President of Home
- Demonstrate an ability to effectively communicate in English, and / or the predominate language of the population being services and must be able to read and
- Pass the Spring Hills Home Care Services Competency Evaluation
- Demonstrates competency in performing all
- Pass a criminal background
- Meet all medical requirements
- Meet all state
- Have reliable transportation and method of communication
- Is punctual, dependable and demonstrates sound
JOB RESPONSIBILITIES :
Follows the established Plan ofAssists with ""Activities of daily living"" means the functions or tasks for self-care which are performed either independently or with supervision or Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.Assists with “Independent Activities of daily living” which may include light housekeeping, shopping, laundry and meal preparation, per the Plan ofAccompany clients to appointments (transportation is client’s responsibility unless driving waiver is signed and per the Plan of Care)Provides assistance with self-administered, pre drawn and medication reminders, per Plan ofUnderstands and maintains infection control and safety measuresReports any and all changes in the client’s condition to the Care Manager orCompletes the Activity Records and accurately documents the care providedPerforms all activities as directed by the Care Manager or BOMHas a positive relationship with clients and familiesHas positive client satisfactionsMeets all deadlines for health screening requirements andEncourage maximum independence to facilitate rehabilitation as determined EP andRespect client’s rights and ensure confidentialityThe CHHA / CNA / Home Care Aide job performance is evaluated 90 days into employment and yearly by Care Manager or BOM using input from clients, coordinators and personalThis job description is used as a guide Each CHHA / CNA / Home Care Aide is responsible for complying with all SHHCS’s policiesEXPOSURE RISK : May come in contact with blood or blood-borne pathogens and will require OSHA training.
