Care Growth Manager
Overview
The Care Growth Manager is responsible for expanding our organization’s reach by building and strengthening referral relationships within the healthcare and senior services community. This role is focused on increasing admissions, service utilization, and overall revenue through strategic partnerships and outreach efforts.
We’re seeking a motivated professional who is skilled at forming connections, identifying opportunities, and positioning our company as the preferred provider of home care in the region.
Key Responsibilities
Partnership Development
Strengthen relationships with hospitals, skilled nursing facilities, assisted living communities, and home health agencies to grow referral opportunities.
Establish new partnerships with professionals who guide care decisions, such as hospital discharge planners, case managers, social workers, financial advisors, and elder law attorneys.
Develop a tailored approach to engaging hospital systems and other high-value partners, ensuring our services are top-of-mind for care transitions.
Growth Strategy & Outreach
Design and execute a local growth plan to increase admissions and service hours.
Analyze referral processes in healthcare settings and identify ways to integrate home care smoothly into patient care pathways.
Actively network within healthcare, senior services, and professional communities to uncover new business opportunities.
Track referral activity, identify gaps, and implement solutions to improve conversion from referrals to active care.
Performance & Reporting
Drive measurable increases in admissions, service utilization, and revenue through partnerships.
Monitor and report on referral source activity, outreach efforts, and results using a CRM system.
Provide regular updates on progress, challenges, and action plans.
Qualifications
Customer service experience required with proven ability to build trust and maintain strong relationships.
Healthcare or senior services experience preferred.
3+ years of experience in business development, sales, or relationship management.
Demonstrated success in growing partnerships and generating measurable results.
Excellent communication, negotiation, and problem-solving skills.
Valid driver’s license, proof of insurance, and reliable transportation required.
Compensation & Benefits
Salary : $60,000 - $80,000 DOE + performance bonus
128 hours of PTO annually
11 paid holidays

Mileage reimbursement (if no company vehicle provided)
Work Environment
This role is primarily community-based, requiring frequent travel to healthcare facilities, professional offices, and events. The position also involves independent work in the field and occasional office time for planning and reporting. Candidates should be comfortable adapting to different work environments, including working from their vehicle when necessary.
Physical Demands
Includes standing, sitting, and walking; occasional lifting of materials under 5 pounds; frequent phone and computer use; and the ability to drive regularly.
