Care Coordinator - KIPP Sharpstown (Job Overview)

Schedule : Monday-Thursday (7AM-4 : 15PM) and Friday (7AM-3PM)

    Key Responsibilities

  • Engage daily with students, parents, and guardians through various communication channels.
  • Collaborate with the SBHC team to implement comprehensive care plans.
  • Coordinate care management through scheduling, referrals, and follow-ups.
  • Track assessments and tests, reporting to the clinical team as needed.
  • Navigate health, educational, and community systems for students and families.
  • Utilize health information technology for effective care coordination.
  • Collect and interpret data to support the health-education connection.
  • Build trusting relationships with students and their families.
  • Communicate effectively and transparently with all stakeholders.
  • Inform families about available health and social services.
  • Prepare and contribute to written communications and reports.
  • Assist with paperwork, documentation, and insurance processes.
  • Maintain care coordination documentation and resource inventory.
  • Perform other duties as assigned to support the team and students.
  • Minimum Qualifications

  • High School Diploma or equivalent required.
  • Two years of experience in healthcare, schools, or community systems.
  • Reliable transportation is a must.
  • Strong organizational and communication skills.
  • Knowledge of community resources and external health systems.
  • Experience working with diverse populations.
  • Ability to collaborate, solve problems, and manage conflict.
  • Proficient in time management and balancing priorities.
  • Experience with electronic health platforms is essential.
  • Fluency in English required; Spanish proficiency strongly preferred.
