This position is responsible for providing guidance, education and support to young people in determining the course of their life course and to plan goals regarding education, vocation, living situation as well as other life skills.
Role and Responsibilities
This position provides case management for the residents, and the primary duties include but are not limited to the following :

Promote healthy boundariesPromote good hygiene and appropriate personal appearance among young adultsReinforce socially acceptable behaviors (ex : manners, eating habits, personal safety)Teach and reinforce thoughtful decision makingTeach and reinforce professionalism in work tasks and employmentAssessing consumers\' history of trauma, strengths and needs.Administer ACLSA
Administer CANS (Child & Adolescent Needs & Strengths)Aware of ACE score and implications on service planningDevelop and monitor Individualized Service PlansCoordinating Services and CollaborationMake necessary referrals to meet consumer\'s health, well-being, education, mental health, and employment and future planning needs (i.e. Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)
Facilitate and / or participate in Team Meetings for all consumers .Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to consumersCollaborate and consult with other providers and entities in the provision of best care services to consumersMonitor the provision of services providedSpecific services to consumersMeet with consumers weekly
Evaluate individual progress and communicate with consumers and other stakeholdersTransport residents to and from service-related activitiesFacilitate life skill activities (ex. conflict resolution, anger management, medication management, tutoring) to improve skills needed to live independentlyProviding emotional support to residents in managing daily lifeCommunicate with resident advisors, program management, and / or therapist regarding services and needs of residentsAudit resident charts as assignedMaintain and record receiptsDocumentation RequirementsProvide resident updates in Case Notes
Complete Incident Reports as needed prior to shift endComplete Monthly Summary Reports and Purposeful Visits in a timely fashionRecord ECEMs in the SHINES portal within 48 hours of contactEmail monthly summaries to CSM and ILP coordinators by the 10th of month following reporting periodEnsure medical / dental / clinical appointments are scheduled on calendarPerformance and ConductMaintain confidentiality of consumer information
Maintain appropriate professional boundariesFollow the CHRIS 180 Code of EthicsFollow mandated reporting requirements of GA CodeSupport and maintain an environment of emotional and physical safety for consumers, staff and other stakeholders alwaysDemonstrate customer service.Always promote positive image of CHRIS 180 programs

3 -5 years of experience working with young adults with mental illness and / or substance abuse issuesBachelor's Degree in related fields such as social work, psychology, education counseling is preferredExperience in writing incident reports is a mustSalary
42k-47k (Depending on experience)

Life Coaches work an average of 80 hours bi-weekly as is required to be on call monthly.The work schedule requires flexibility to meet the needs of the program and availability on an emergency basis. Primary hours are between 9 : 00am-6 : 00pm.Due to schedule or other operations necessities, the organization may transfer an employee to another location or shift, either on a temporary or permanent basis as this position is an employee of CHRIS 180, not the program.Physical Demands
