Wellstar North Fulton Hospital has an opportunity for a Manager- Care Coordination. Full-Time Day Shift

Responsible for provision of hospital level leadership and management of all operational processes in the Care Coordination Department. Manager is responsible for implementation of evidence-based standards of practice, compliance with system Care Coordination policies and procedures, and for maintaining and improving the quality of care coordination delivered to all patients. Manager is responsible for assuring staff and program compliance with regulatory standards and requirements, and for monitoring team member engagement and advocacy with patients and families. The Manager works with the Director to assure timely reporting and coordination of all financial and strategic performance initiatives.

Core Responsibilities and Essential Functions

Care Coordination Leadership

• Oversees team member work in the discharge assessment and planning, from admission to discharge.
• Assists Director in the implementation and coordination of the Hospital Length of Stay Committee.
• Develops and maintains evidenced based care coordination standard work order sets and care paths and updates annually to reflect evidence-based care coordination practices.
• Analyzes, interprets and makes recommendations based on care coordination outcome data.
• Monitors adherence to standard work, tracks variances from protocols / care paths and reports data to team and organization.
• Serves as a liaison to hospital clinical departments by providing feedback on care coordination outcomes and analyzing individual dept. performance with regard to established goals.
• Ensures compliance with relevant CMS and commercial payor standards to maintain specialty contract compliance and completion of all intra-cycle reports to be submitted to payors within required timeframe

Required Minimum Education

Masters Social Work or Bachelors Nursing or Masters Nursing-Preferred

Required Minimum License(s) and Certification(s)

All certifications are required upon hire unless otherwise stated.

Additional License(s) and Certification(s)

Certification in Care Management by National Accrediting Body Required

Required Minimum Experience

Minimum 5 years recent care coordination experience in hospital setting Required and Minimum 2 years recent experience in management of a multidisciplinary staff in the field of care coordination Required

Required Minimum Skills

Strong interpersonal skills to facilitate team dynamics are necessary Strong verbal and written communication skills Time management skills, facilitative leadership skills, prioritization skills and change management skills are necessary to focus on goal achievement for the system
