There is an open Part Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Twin Falls, ID. The Part Time Nurse Practitioner (NP) will conduct home health risk assessments within Twin Falls County ID. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Twin Falls, IDand the surrounding Twin Falls County ID.Twin Falls County ID includes locations like Kimberly, Buhl, and Filer.Candidates could be scheduled in any one of these locations during an individual shift.
Responsibilities :
- Conduct annual home health risk assessments for a Medicare Advantage health plan
- Evaluate prior and current medical history, medical conditions and medication history
- Provide education on medical conditions and medications
- Identify any gaps in care and provide recommendations for resolution
- No creation of treatment plans or prescribing of medications is required
