The roles require critical thinking, attention to detail and being able to prioritize appropriate interventions. An essential bridge between the assessment coordinator, inter-departments and external stakeholders that include, but is not limited to the Department of Community Health, health care providers, and medical institutions.\n\nThis role performs frequent analysis, excellent tracking skills, organizational skills, analytical and exceptional communication skills. Must always be committed to providing responsive aid courteously and professionally.\n\nPrimary Duties and Responsibilities : \n
• Assess clients in their homes for Elderly and disabled Waiver Programs.\n
• Provide full MDS assessment for new and renewed members.\n
• Responsible for assisting with review of member assessments, implementations, and management of nursing services within the ACE Care scope.\n
• Provide disease management initiation and updates as needed\n
• Responsible for assisting with the initiation, development and maintaining plan of care.\n
• Assist in Identifying family / client needs and for providing supportive care in accordance to the plan of care.\n
• Perform education to maintain standard of care within the nursing and case management department.\n
• Maintain regular communication with Administrator, other RN and LPN staff and lend assistance concerning client's progress and problems.\n
• Follow up with compliance for physician's orders and treatments being provided by direct care providers\n
• Assist with provider meetings as needed\n
• Provide support and participate in IDT weekly meetings\n
• Responsible for knowing and understanding policies and procedures that govern the EDWP formally the SOURCE program.\n
• Accepts other assignments as appropriate.\n\nRequired Education and Experience\n
• RN / LPN active licensure\n
• At least three (2) years in a related field preferred\n
• MDS experience is preferred but not required. We will train!\n
• Must be knowledgeable in general nursing standards of care and medical practices.\n
• Excellent analytical and deductive reasoning skills\n
• Computer literacy and comprehensive understanding of electronic documentation programs\n
• Willingness to Travel\n
