Registered Nurse Job Details Location : Rutland, VT 05701 (Vermont nursing jobs) Assignment Duration : 13 weeks (10 / 27 / 2025 – 1 / 26 / 2026) Hours per Week : 40 (Guaranteed 37 hours) Shift : 8-hour days (Mon-Fri 8am–4pm with some weekend / on-call required, or Tues-Sat, or Sun-Thur with some on-call) Employment Type : Travel RN, On-Site Estimated Weekly Salary : $2100 - $2250 Homecare RN Qualifications Active Registered Nurse (RN) license in Vermont or compact state eligibility Minimum 1 year of recent experience as a Home Health RN Proficiency with OASIS documentation (required for home health nursing jobs) Strong clinical skills : IV use / maintenance, wound VAC, complex wound care Ability to work independently and manage a homecare caseload Excellent communication, organizational, and documentation skills Home Health RN Responsibilities Assess patient and caregiver needs to deliver high-quality, evidence-based homecare in compliance with state and federal regulations Develop and communicate individualized plans of care, engaging patients and caregivers Serve as Case Manager for assigned home health patients, efficiently managing caseloads and visit schedules Complete OASIS documentation accurately and promptly Implement medical plans of care, nursing interventions, and procedures for safe, effective patient care Maintain infection control standards and stay current on advanced clinical skills Collaborate with LPNs and interdisciplinary healthcare team members to ensure comprehensive patient care Educate patients and caregivers on medications, treatments, and discharge instructions Coordinate care with physicians, providers, and other healthcare disciplines Participate in peer review audits, case conferences, and interdisciplinary team (IDT) meetings Ensure timely and accurate completion of agency, state, and federal documentation Support safe patient discharges and transfers, ensuring all required documentation is complete Ready to make a difference as a Homecare Registered Nurse in Rutland, Vermont?
